Quality CMS Update
The CMS Hospice Quality Reporting Program (HQRP) continues to expand and include new measures and requirements. The two new measures, Hospice Visits in the Last Days of Life (HVLDL) and the Hospice Care Index (HCI) come from claims and are now available in CASPER. Star ratings are being implemented for the Hospice CAHPS ® survey. How data is presented on CARE Compare is expanding and the HOPE tool continues to be tested by a group of hospices. Prepare for change: Making Revenue Cycle a Priority
With the introduction of new value-based payment models, hospice providers will need to form new payor relationships. MA Hospice Carve-In is anticipated to bring downward pressure on reimbursement and a subsequent decrease in margins. The billing of hospice services through the traditional Medicare will change dramatically. Claims management will need to be transformed and transparent to ensure cashflow is not interrupted and denials do not increase.

Advance Illness Management through Home Care Innovation
This session will present an innovative care delivery model called Advance Illness Management (AIM), intended for patients with serious illnesses needing complex care and/or end-of-life care. AIM incorporates the use of televisits by physicians and CRNPs that are enabled by home health nurses. The program led to improved outcomes, notably: lower ED visits and hospitalization rates (decrease by 50%), higher hospice use (increase by 48%) with goal concordant care, and lower cost.

Standardizing Home-Based PC with Payers & Providers
Consensus-based standards for home-based palliative care were developed and implemented with multiple payers and providers in California. Never Waste a Crisis: Making a Traveler Proof Program
The experience of a rural hospice whose plan for radical care model redesign was complicated by unplanned leadership changes, staff resignations, increased patient demand, a pandemic, and soaring travel nurse costs will be used to discuss lessons of survival and change. The application of human factors engineering (HFE) and principles of high reliability are providing stability and an opportunity to create a care system capable of consistent quality while relying on travel nurse staff.

Triangulating Outreach, Education, & Clinical Care
This session addresses the gap in healthcare for those individuals living with serious illness through outreach, education, and clinical care. Faculty will present a collaborative model of the Center for Research on Health Disparities, Appalachian American Alliance of Nurse Practitioners, and Embedded Palliative Care Clinician at Clemson University. Through triangulation, providers reduce inequity and support the needs of those with serious illnesses. This model expands palliative care services into marginalized, rural communities in South Carolina.
