Tuesday, Sep 20, 2022	Tuesday, Sep 20, 2022 3:00 PM - 4:00 PM EDT	Dependence to Interdependence: Empowering Patients As well as care team members aiming to understand our patients and their ever-changing needs, it is impossible to be their eyes and ears at all times, and that's where patient empowerment can help. This session presents a model for home-based palliative care focused on empowering patients to understand their disease process, recognize impending problems or disease exacerbation, and intervene before their condition deteriorates without needing to wait for a provider appointment. We will dive into the development of program parameters, patient toolkits, and telephone triage systems to implement this model. Attendees will learn how to help patients become an active member of their own care team, able to assist providers with pertinent, accurate, and timely information gathering and optimize the self-management of their chronic conditions.
