Dependence to Interdependence: Empowering Patients
As well as care team members aiming to understand our patients and their ever-changing needs, it is impossible to be their eyes and ears at all times, and that's where patient empowerment can help. This session presents a model for home-based palliative care focused on empowering patients to understand their disease process, recognize impending problems or disease exacerbation, and intervene before their condition deteriorates without needing to wait for a provider appointment. We will dive into the development of program parameters, patient toolkits, and telephone triage systems to implement this model. Attendees will learn how to help patients become an active member of their own care team, able to assist providers with pertinent, accurate, and timely information gathering and optimize the self-management of their chronic conditions.

Impact of Moral Distress in the Workplace
Moral Distress is defined as knowing the right thing to do yet feeling unable to do it. For hospice and palliative care professionals, this phenomenon was only intensified as the global pandemic changed the landscape of care delivery. In this session, we will explore these challenges with the NHPCO Ethics Advisory Council and share resources created to help address the Moral Distress that end-of-life care professionals face.

Ready! Set! Role Play! Infusing Empathy Into Your Work
As hospice and palliative care providers, our days often entail challenging conversations with clients, co-workers, patients, and caregivers, but it may be diffcult to determine how well we are communicating during these conversations. In this session, select participants will have the opportunity to practice these difficult conversations in a safe and low-stakes environment with a professional role player. Observing participants will provide live feedback and all attendees will walk away with tools they can immediately implement to improve their communication skills.
