Solving The $PD Production/Quality Challenge
Are you interested in learning more about the cost of SPD– and OR-related instrument errors and how a robust quality system focused on performance improvement will lead to reduced errors, improved patient safety and better morale for both the SPD and OR? Every SPD has three primary goals: Instruments must be available when needed, functional and safe for patient use. Failure to achieve any one of these critical goals can place a patient at risk and cost the healthcare system valuable resources of time and money. To achieve these goals, the SPD has four variables with which to work. Production is a function of the number of staff, the staff's proficiency, equipment and the efficiency of the department. Deficiency in any one of these variables will negatively impact the SPD's ability to achieve its three primary goals. Additionally, unless the OR is cooperative in proper handling and point-of-use treatment, the SPD's production will be negatively impacted. Instrument-related events caused both by the SPD and OR are estimated to cost the healthcare system over $1B annually. To measure performance and the cost of these errors, a robust quality system is needed.

Breaking Down Silos: Bridging the Gap Between IP and SPD
Picture for a moment two roles in your healthcare organization. One is the Sterile Processing professional working to clean, assemble and sterilize instruments for the next surgical procedure; the other is the infection preventionist who monitors emerging pathogens, audits compliance—and when something goes wrong, determines what can be done to keep it from reoccurring. It is not uncommon for Sterile Processing and Infection Prevention to both work in silos where communication and collaboration are limited. With a shared vision for best practices and passion for education and collaboration, the bridge between IP and SPD can be strengthened to ensure safe patient care.

Cleaning up the Department: The Journey to Becoming Compliant with the ST79 Standard for Environmental Services
Session participants will learn about ANSI/AAMI ST79, Amendment 1, and be able to discuss the importance of optimizing the Environmental Services (EVS) partnership and program in Sterile Processing. The presenters will describe challenges and successes with their current program and how they achieved success by complying with the current standards—and with a visibly cleaner department by partnering with Sterile Processing leadership, Infection Prevention and EVS through a QR-code tracking system. The presentation will also explain how to add surface monitoring as a quality improvement check, in partnership with the EVS leadership, and how to use it to improve the process.

Identification and Prevention of Surface Alterations on Surgical Instruments Caused by Waterborne Minerals
During medical device processing, unwanted surface alterations and discolorations may occur and may arise from waterborne minerals during the decontamination process. Discolorations from steam sterilization may occur when the water used for feeding the steam generator has waterborne minerals and metal ions. To gain a deeper understanding, instruments with characteristic discolorations from different hospitals were used for further studies on the chemical nature of these discolorations. Analytical methods used included energy dispersive X-ray-spectroscopy (EDX) and X-ray photoelectron spectroscopy for chemical analysis (ESCA). Further physical investigations were made to explore the behavior under mechanical stress.

Keep Them Growing! Sterile Processing Professional Development and Retention
Sterile Processing (SP) leadership, under the direction of Whitney Huddleston, recognized an opportunity to train and elevate employment opportunities by focusing on professional development of technicians and leadership. A multidisciplinary team established an internship to train new SP technicians, developed an SP-focused preceptor course, established a career ladder, and are currently partnering to develop a simulation lab to better assist education of staff through situational simulation. This presentation will explain how these steps have improved SP staffing, increased knowledge and productivity, while also boosting retention through professional development opportunities.

Sterilization of Flexible Endoscopes
Current sterilization methods for reusable medical devices include steam, ethylene oxide and vaporized hydrogen peroxide. Because flexible endoscopes are heat sensitive, steam sterilization is not suitable, making low-temperature sterilization more viable; however, each of these low-temperature modalities has advantages and disadvantages in terms of material compatibility, throughput, efficacy and standardization. Due to technological limitations, none of these methods alone can be universally implemented. This presentation explores how increasing demands from healthcare facilities and advancements in both endoscope material design and sterilization technology can make low-temperature sterilization methods practical for reprocessing of flexible endoscopes.

The Science Behind Sterilization Monitoring
This presentation delves into the science and technology behind sterilization monitoring indicators, why they are important, and how they are designed to indicate whether different parametric release metrics have been met. In any healthcare facility, healthcare-associated infections are a concern. Regulations require sterilization monitoring to ensure the sterilization equipment is functioning properly. Several types of sterility monitors exist, but what are the differences? Are biological indicators (BIs) really better than chemical indicators (CIs)? What do they all indicate? Learn how the chemistry behind the indicators allows for control and specificity to sterilization cycles. There will be a focus on CI technology as well as how the indicators perform relative to BIs.

I.D. Please! Casket/Basket Instrumentation Identification Process After Tape Removal
To align our organization’s practices with the Joint Commission’s focus on patient safety, UMass Memorial Medical Center (UMMMC) developed a plan to remove identification tape from all surgical instrumentation. Procedures, timelines and goals were created and implemented. With collaboration from our repair vendor, our Sterile Processing teams were able to accomplish full tape removal on over 12,000 surgical instruments. To avoid a break in service, these teams created the “casket/basket” instrumentation identification system. This created a safe and organized process for our staff to continue to service the departments and clinics of UMMMC. The understanding that there would not be a gap in service allowed for increased buy-in from these outside areas. Removing tape from all surgical instrumentation provided higher-quality patient care by reducing the risk of infection caused by tape. Reducing patient harm caused by medical devices is a pillar of our process improvement initiatives. Our responsibility as healthcare workers and as an organization is to consistently provide the highest quality patient care.

Manual Cleaning of Endoscopes: Looking Ahead to the Next Standard of Care
Flexible endoscopes have been associated with the greatest number of patient infections when compared to other reusable medical devices. The body of evidence shows that patient-ready endoscopes can remain contaminated despite undergoing high-level disinfection or sterilization, warranting review of upstream processes. Ineffective manual cleaning of the inner channels is one of the major root causes of biofilm formation, which contributes to failed disinfection. The presenter will review the evidence associated with manual cleaning challenges, review the efficacy of a next-generation technology for addressing these issues and outline the implications for reprocessing practices.

Using a Human Factors and Ergonomic Approach to Infection Prevention in Sterile Processing
Human factors engineering allows us to assess our capabilities and limitations within our work environment and workflow processes. There is a continuous need to monitor factors that impact human performance and eliminate adverse events to ensure patient safety. This can be accomplished by assessing the complexity of roles and workflow, risk perception and level of competency as well as the organizational commitment to safety. Visual Inspection Beyond Endoscopes: Suctions, Shavers and Probes
Standards, guidelines and peer-reviewed research have highlighted the need for careful visual inspection of endoscopes, but inspection of other types of medical instruments has received less attention. In this presentation, three experts will discuss investigations of surgical instruments and ultrasound probes. Presenters will show photos and videos demonstrating notable characteristics of these instruments that could impact cleaning effectiveness. They will also discuss defects observed in fully processed items and the effectiveness of attempted solutions. Further, they will provide insights for developing quality management initiatives that incorporate visual inspection, and discuss approaches for engaging stakeholders, acting on findings, and managing risk.

Working Together— A Focus on Water and Steam Quality
The core of any water management program is based on industry best practices from ANSI/AAMI ST108 and ANSI/ASHRAE standard 108 is the participation and leadership of a cross-departmental team. Establishing this team is critical for the success of any program, providing for different viewpoints, expertise, responsibilities and accountability to the program’s success. This, however, is often a challenge with departments that generally do not work together. Intentional Mentoring in the Sterile Processing Environment
When it comes to new employees in the Sterile Processing department, there will be a variety of skill levels coming into the area. This can range from someone who knows nothing of the job, to students who have some knowledge of the skills, and seasoned technicians who need to learn your hospital's policies. With these different groups comes a different set of skill sets that need to be taught. Participants will learn how do discern the differences and be intentional with their training of each group. We will discuss the different types of people encountered when precepting and address how their skills needs differ and what can be done to train each group intentionally and most effectively—an approach that will also benefit preceptors and others in the department.

Rolling the Dice: Small Changes, Big Impact
“Rolling the Dice” by making small changes greatly impacts both SPD and the OR. Improving the culture in SPD and ORs is vital both for ensuring quality, safety, outcomes, and efficiencies. Adding small changes cannot only foster relationships between the two groups; these changes can impact retention, recruitment, and staff satisfaction. The presentation will be to share with the attendees the journey of taking a team with 19% of organizational vacancies in 2021 and reducing it to 3.6% by the second quarter of 2023. This journey faced numerous challenges, including “ghosting” by staff members and new hires.
