Please be advised that to increase accuracy, take your time and answer each question. It can take up to five minutes to fill out this form. Please allow yourself time to read every policy before beginning.

Health & Safety Update: Vaccinations are no longer required to attend the 2026 Women's International Convention. Attendees are strongly encouraged to be vaccinated. We are adhering to CDC guidelines, including the use of masks, social distancing, and other protocols while inside all convention venues.
