This gathering offers a dynamic opportunity to stay ahead of industry trends, exchange insights, and collaborate to drive meaningful change within your organization and across the care at home community. With content designed for leaders at every level, the Annual Meeting addresses current challenges while anticipating those on the horizon.

The Alliance Education Workgroup is seeking proposals that present innovative ideas, best practices, or case studies delivered in engaging formats. Submissions that emphasize practical strategies, real-world applications, and inclusive perspectives are highly encouraged.

This conference will focus on advancing patient-centered solutions across the full continuum of home-based care, including home health, hospice, Medicaid Home and Community-Based Services (HCBS), palliative care, and home care.
