Facial aging is a complex process that involves changes in multiple tissue layers, leading to alterations in facial contour, shape, ratios, and proportions. Starting around age 25, fibroblasts produce less collagen, resulting in reduced skin elasticity. In the fourth decade of life, fat repositioning becomes noticeable, contributing to midface sagging and a more gaunt, tired appearance. This is further aggravated by the loss of superficial fat, which accentuates contour irregularities. After age 50, changes in the craniofacial skeleton reduce the support for facial ligaments, leading to further sagging. The 7-point technique with PLLA offers comprehensive facial rejuvenation by addressing these architectural changes across all three tissue layers, helping to slow down the progression of skeletal changes.

Frontal concavity significantly impacts the aging appearance of the upper third of the face. This concavity results from a loss of soft tissue volume on the forehead, leading to brow ptosis and furrowed eyebrows. These changes can convey expressions of anger, tiredness, and sadness, and in women, they can masculinize the forehead, reducing its feminine appeal. Frontal concavity varies by gender and genetics. Women typically have high-arched brows and an open eye appearance, emphasizing the upper third of their face. In contrast, men often have less soft tissue volume in this area, resulting in more pronounced frontal concavity. Frontoplasty, utilizing hyperdiluted hyaluronic acid, is an effective method to address these forehead changes.
