Closing Symposium: Built to Last: Decision Making for Implant Placement in Medically Compromised Patients
In implant dentistry, we often say “dental implants must be prosthetically driven” but is this statement incomplete if it does not consider patient as a factor in implant placement? Indeed, despite the ubiquity of dental implants in our environment, challenges remain due to the high prevalence of peri-implant disease, esthetic concerns, and myriad surgical protocols. As dental healthcare professionals, we aim to limit risk and optimize outcomes for each patient we see. This course seeks to aid dental healthcare providers in recognizing medical risks that may impact patient outcomes for dental implant placement and to develop protocols that weigh and the risks and benefits for each of our patients so that we can deliver treatments that are "prosthetically driven AND biologically executed.”

Closing Symposium: Use of Digital Technologies and Artificial Intelligence to Optimize Implant Therapy
The era of digital dentistry has exploded in the last 20 years and has fundamentally changed our profession. However, it has also come at a price which questions our biologic compass as treatment has become heavily predicated on computer-guided expectations compared to decision making based on surgical experience and core treatment planning principles. From each full volume CBCT scan, a historical picture of growth and development can be gleaned, influencing our decision making. To that end, each treatment plan we put forth and the materials we choose to alter or influence bone can have consequences on the healing dynamics, the esthetic/biologic outcomes realized and, ultimately, our successes in creating sustainable conditions. Management of Peri-implant Esthetic Complications: Peri-implant Soft Tissue Dehiscences: Classification and Treatment
The recession of the buccal soft tissue margin is a frequent complication of well-integrated dental implants. The appearance of metallic structure or even their transparency through the thin buccal soft tissues are common reasons for patient aesthetic complaints. Moreover, bad implant installation frequently results in excessive apical dislocation of the buccal soft tissue margin of the implant-supported crown. Soft-tissue, plastic surgical procedures and bilaminar techniques in particular, can be successfully used in combination with a pre- and post- surgical prosthetic approaches to increase the volume of the interdental soft tissue, to treat buccal gingival recessions and soft tissue dehiscence around dental implants and to provide the new implant supported crown with an esthetic transmucosal emergence profile.

Immune Etiology
The immune system reacts to a foreign material such as titanium and produces bone tissue around the implant to protect nearby tissues. Osseointegration is an immune response. The immune system is likewise behind marginal bone loss (MBL) around oral implants through its control of the osteoblast/osteoclast combined action. The immune system protects the implant against bacterial attacks. Numerous factors related to the implant and the patient as well as to clinical handling combine to cause MBL which is generally an aseptic reaction. If the combined factor attacks are very strong, the immune system may surrender with the result of implant failure. with a combined aseptic and septic attack on osseointegration.

Prosthodontic Considerations to Prevent Peri-implant Diseases
Over the past decade, emerging evidence indicates a strong relationship between prosthetic design and peri-implant tissue health. An ideal implant position is one of the most critical factors in achieving an acceptable implant restorative design. Malpositioned implants often result in a restorative emergence profile at the implant-abutment junction that can restrict patients' access to adequate oral hygiene. Inadequate cleansability and poor oral hygiene have been reported as precipitating factors to induce peri-implant mucositis and peri-implantitis and are influenced by restorative contours. The implant-abutment connection, restorative material selection, and restoration design are also reported in the literature as potentially affecting peri-implant soft tissue health.

Microbial Etiology
Dental implants replace missing teeth in at least 100 million people, yet over one million implants fail every year due to peri-implantitis, a bacterially induced inflammatory disease. Our ability to treat peri-implantitis is hampered by a paucity of information on host-microbiome interactions that underlie the disease. By characterizing molecular events at the mucosal-microbial interface in the peri-implant crevice, we discovered an active peri-implant microbiome surrounding healthy implants. Microbial genes regulate human genes encoding cell development, metabolism, morphogenesis, adhesion, gap junctions, cell-cell signaling and immuno-inflammatory pathways, suggesting a role for commensals in protecting epithelial integrity. In disease, we discovered that microbial dysbiosis in the peri-implant sulcus promotes abandonment of host-bacterial transactions that dictate health and instead drives a move towards chronic programming of a non-healing wound.

How to Prevent and Manage Peri-implantitis Using the Experience Gained From Treating Implant-associated Chronic Osteomyelitis
Implant-associated osteomyelitis is the bane of elective orthopaedic surgeries (i.e. total joint replacement). Staphylococcus aureus is the primary pathogen, and 50% of cases involve MRSA. I will present experimental evidence with clinical correlates demonstrating four distinct pathogenic mechanisms: intracellular infection, biofilm on implants and necrotic tissue, staphylococcus abscess communities and colonization of osteocyte lacuno-canalicular network (OLCN). I will also present pre-clinical and clinical data on the immune proteome against S. aureus, and that both protective antibodies and pathogenic antibodies exist. Protective antibodies (e.g. anti-glucosaminidase) neutralize critical S. aureus proteins, while pathogenic antibodies (e.g. anti-IsdB) facilitate Trojan horse leukocyte formation, bacteria dissemination from surgical site infection, and septic death.
Based on these basic science discoveries I will present pre-clinical research on bone targeting antibiotics, which overcome the pharmacokinetic and biodistribution limits of standard of care antibiotics, and have potential to kill bacteria within the OLCN of cortical bone. I will also present pre-clinical data on a passive immunization for MRSA. As there are many parallels between orthopaedic implant-associated osteomyelitis and peri-implantitis, I will conclude with thoughts on how this new information might be used to prevent and manage oral infections. Metallosis and Peri-implant Diseases
The majority of dental scientists and clinicians believe that the primary etiology of dental implant loss is related to oral biofilm. In fact a number of recent publications indicated that this bacterial mass is the sole etiologic factor in the vast majority of cases of peri-implant mucositis and peri-implantitis. However, there is a growing body of evidence that implicates small particles of titanium and zirconium in the etiology of the inflammatory response seen in these cases. This process is termed metallosis. It was first discovered around failing prosthetic knees and shoulders. Emerging evidence has linked these particles to inflammation around dental implants. This lecture will cover the current understanding of this phenomenon and how it relates to dental implant failure as well as appropriate steps to avoid and treat the problem.

Surgical Track: Mucogingival Approaches for the Management of Peri-implant Disease
In the beginning, osseointegration was thought to be sufficient for long-term successful implant rehabilitation. With time, it became clear that soft-tissue integration is of paramount importance for long-term success. Recent studies suggest that the absence or a reduced width of KM may negatively affect self-performed oral hygiene measures, and that poor plaque control constitutes a risk factor for peri-implantitis. As a result, in cases of peri-implant biological complications, the treatment should also be focused on the creation of an effective soft-tissue barrier capable of biologically protecting the peri-implant structures.

Surgical Track: Strategies for Predictable Bone Augmentation Leveraging Tissue Engineering Strategies
Current treatment modalities to restore lost alveolar bone tissues can provide functional and structural restoration of the compromised or lost tissues; yet, many of these approaches do not meet the need for more biologic and predictable treatment outcomes. Historically, the use of different biomaterials and grafting techniques has established a strong platform for the development of regenerative medicine concepts toward achieving “true“ tissue regeneration. As part of dental implant therapy, alveolar bone grafting is considered standard-of-care for patients with alveolar bone deficiencies. Though routinely performed, these grafting procedures still have limitations. This discussion will describe innovative and emerging personalized medicine and tissue engineering strategies for reconstruction and regeneration of alveolar bone tissues. Key regenerative medicine principles will be discussed and clinical cases will be presented demonstrating how tissue engineering strategies can be implemented in clinical practice.

Surgical Track: Strategies for Predictable Vertical Bone Augmentation
Patients with missing teeth in the “esthetic zone” may present with severe hard and soft tissue loss. While many techniques offer excellent results for reconstruction of these defects, few can be said to guarantee success. A bone augmentation procedure to correct a vertical defect in the esthetic zone that is not fully successful in correcting the defect often ensures the use of prosthetic pink. This leads to the treatment dilemma: “if we have to use a little pink, why not use a lot of pink” and spare the patient the painful bone augmentation in the first place? The aim of this lecture is to critically evaluate the current evidence to determine the predictability of various bone augmentation techniques for vertical ridge defects for implant placement and when to consider more conservative alternative treatment options.

Surgical Track: Implantoplasty in the Surgical Treatment of Peri-implantitis
Many of the peri-implantitis lesions treated today are complex in their bone morphology as the remaining walls surrounding the affected dental implant are not always circumferential i.e. clot/graft containing in their configuration. Moreover, the disease process may also leave portions of the implant body supracrestal to any remaining lesion. This is due to a variety of factors including the position of the dental implant. In those areas where regenerative treatment is not possible, it may be best to decontaminate the surface with implantoplasty and manage the remaining lesion through resection. The goal is to establish heath with cleansability. On the other hand, there may be a number of combination lesions where a hybrid approach i.e. regeneration with resection should be provided. WAIT LIST - SOLD OUT After implant insertion, especially in the mandible, it is common to face clinical scenarios in which there is complete absence or a minimal quantity of keratinized tissue, as many times this is associated with a reduced depth of the vestibular fornix and a coronal muscle insertion. In these cases where there are no esthetic demands, the main objective is to reconstruct/increase the quantity of keratinized mucosa and deepen the vestibule so as to facilitate the performance of oral hygiene by the patient and at the same time reduce the risk of soft tissue dehiscence . The most appropriate surgical technique to obtain those results is the free gingival graft. Most of the time, soft tissue deficiencies at the buccal aspect of implant-supported crowns are a matter of thickness rather than height. The lack of adequate thickness of the buccal soft tissues can lead to incorrect emergence profiles of the prosthetic crown, which are difficult to maintain from a hygienic point of view. In these cases, the main goal of plastic surgery is not to increase the height of the keratinized tissues but to augment the thickness of the connective tissue at the level of the peri-implant transmucosal portion. This is achieved with a surgical technique that involves the use of a connective tissue graft covered by a coronally advanced flap. Peri-implant diseases are emerging with tenacious complications. The etiology is unclear with unpredictable treatment outcomes. Millions of patients with dental implants are haunted and threatened by these diseases. Through clinical practices and experiences with the operating microscope, we have this unique opportunity to evaluate the etiologic factors and perform precise tissue management under higher magnification and with micro-instruments. The modified microsurgical treatment protocol has led to favorable preliminary outcomes. Minimally invasive soft tissue management, implant surface decontamination, and biomaterial stabilization might have contributed to these positive regenerative outcomes. Non-invasive and point-of-care ultrasound imaging of peri-implant tissues of diseased implants provides not only anatomical information important for clinical decision making but also functional information possibly related to wound healing and prognosis. Bony defect topography, soft tissue dimensions and quality can be evaluated on ultrasound. Quantitative tissue perfusion analysis from ultrasound could shed light on understanding of regenerative wound healing of treated implants. The quality of socket healing that may relate to implant prognosis and longevity could be evaluated by ultrasound, based on our recent study. This presentation will cover fundamentals about the use of the operating microscope for managing peri-implant diseases and new findings from ultrasound research on peri-implant diseases.

Team Program: Don’t Have a Screw Loose! Supportive Dental Implant Therapy: Current Evidence and Protocols
Implant-related complications are common and difficult to manage. Many patient-, site-, and fixture-related factors convey risk of future peri-implant diseases and must be considered as a part of developing patient-centered implant maintenance plans. Such ongoing peri-implant maintenance protocols are critical for long-term implant success and survival. For these reasons, implant maintenance and patient-delivered home care that includes implant evaluation and delivery of supportive therapy in a individualized, risk-based schedule should be performed. This presentation will focus on common clinical scenarios and the mechanisms to employ primary prevention of peri-implant diseases through proactive, evidence-based implant maintenance.

Single dental implants have now been used in the esthetic zone for over thirty years. There exists a large cohort of patients with well-integrated dental implants who desire esthetic improvement of their restoration after decades of service. Restoration of the dental implant represents a clear challenge for the dentist. Not only immediate, such as what type of connection, cement or screw retained, the material to use, but also how to best implement the tools we use such as torque wrenches. Beyond the initial restoration goes the challenge of maintain the site- evaluating for disease- what tests to use and how useful they are. Prosthetic Track: Full-arch Rehabilitation: Removable Versus Fixed
The primary objective of an immediate loading protocol is to minimize the need for multiple surgical procedures and expedite the time between surgery and prosthetic delivery, all while maintaining high implant success rates and achieving optimal functional and aesthetic outcomes. Immediate loading protocols can effectively alleviate patient concerns and significantly enhance the acceptance of implant therapy. During the lecture, we will provide a step-by-step demonstration of two novel full digital protocols for fully edentulous and transitional patients: the Simplified Digital Protocol (SDP) and the Minimally Invasive Full Arch Protocol (MIFA).
